Rooibos and Breast Cancer, What a Lab Study Found

A Stellenbosch University lab found that rooibos extract steers estrogen toward the receptor that suppresses tumor growth rather than the one that drives it, in breast cancer cells grown in a dish. The researcher who found it is the first to say what that does not yet prove.

What the laboratory actually did

The question arrives at the Ministry with some regularity, and it deserves a straight answer before anything else: no. Rooibos does not treat breast cancer, and nothing published so far suggests it prevents it. What happened is smaller than the headlines that carried it, and more interesting than most of them managed to convey. In a laboratory at Stellenbosch University, extracts of the red bush were introduced to estrogen-driven breast cancer cells growing in culture, and those cells behaved differently than they otherwise would have. That is a finding about cells in a dish. It is not yet a finding about you, or about the cup cooling beside you, and the biochemist who made it has been unusually firm on exactly that point.

The work comes from Dr Nicky Verhoog, a senior lecturer in biochemistry at Stellenbosch1, whose university is known locally as Maties. It was supported by the South African Rooibos Council and the Department of Science, Technology and Innovation through its Sector Innovation Fund Programme, and the Council is funding the next phase. The results became public in early October 2025, around South African Breast Cancer Awareness Month, which is a season that tends to enlarge a modest finding on its way through the news. News24 reported the same result3 at the time, so the underlying study is real and consistently described. It is the interpretation, not the science, that needs a steady hand.

In the dish, the extracts slowed estrogen-induced growth of the cancer cells and prevented them from spreading. Dr Verhoog's own summary is careful about where the sentence ends: rooibos, she said, "appears to act in a protective way against oestrogen-driven breast cancer cells in the lab"2. The last three words are load-bearing, and the Ministry would ask you to carry them along with the rest.

Two receptors, one saying go and one saying stop

To follow the mechanism you need only one idea, and it is not a difficult one. Estrogen is a signal. A receptor is the lock that signal fits into, and the consequence depends on which lock is opened. Breast tissue carries two of interest here. Estrogen receptor alpha, ERα, is the one associated with driving tumor growth. Estrogen receptor beta, ERβ, is associated with suppressing it. The same hormone, arriving at two different doors, produces two opposite instructions.

What the Stellenbosch group observed is that the rooibos extracts preferentially activated ERβ rather than ERα. In other words, of the two available doors, the extract kept opening the one associated with restraint. In this specific respect the extract's action resembled fulvestrant, a standard estrogen-blocking drug used in breast cancer treatment.

That comparison is the sentence most likely to be misread, so let it be pinned down. A resemblance in one measured behaviour, in one cell line, in one laboratory, is not equivalence. Fulvestrant is a purified compound given at a controlled dose under medical supervision, with decades of trial evidence behind it. An extract in a culture dish is a research tool. Neither one is a mug of tea, and the concentration a cell sees in a laboratory bears no fixed or knowable relationship to what reaches a person who has had two cups after supper.

The tamoxifen finding, and why it matters most

There is a second result in the work, and for a certain reader it is the practically useful one. Many women on endocrine therapy have wondered, quietly and often without asking anyone, whether their daily rooibos might be working against their medication. In the laboratory, the rooibos extract did not interfere with the effectiveness of tamoxifen2, a standard breast cancer therapy. The early evidence, such as it is, does not point to a conflict.

This is in fact closer to the study's actual purpose than the protective-effect headline was. As Dr Verhoog put it, "what this research is doing is testing its safety, specifically in women with oestrogen-sensitive breast cancer"1. Safety, not cure. The question on the bench was whether a widely consumed national drink causes a problem for women with hormone-sensitive disease, and so far, in cells, it did not. That is a reassuring absence rather than a promising presence, and the two are easy to confuse. Anyone actually on treatment should take the question to their oncologist, who knows the case; the Ministry keeps records, not patients.

The distance between a dish and a body

The in vitro study is complete. The next step, announced as planned for 2026, is animal model trials, which exist to answer a question cell culture structurally cannot: what happens to the extract once a living body absorbs it, metabolizes it, distributes it, and disposes of it. As of this writing there is no public confirmation that those animal trials have begun. Human trials are not on the horizon, and would be several years and several funding cycles beyond the animal work even in the most favourable case.

Dr Verhoog's own caution is worth carrying in full rather than softened into paraphrase. Lab studies, she said, "involve single cell types and don't replicate the complexity of the human body."2 The reporting on her work puts the limit plainly: these findings should not be read as evidence that rooibos can prevent or treat breast cancer. A dish contains one kind of cell and a controlled dose. A person contains a digestive tract that transforms compounds before they ever reach the bloodstream, a liver with opinions, an immune system, and a great many other cell types all in conversation. Most compounds that look interesting in culture do not survive that journey. That is not pessimism, it is the ordinary attrition rate of early research, and it is why the animal stage exists.

What is fair to say over the kettle

None of this changes what rooibos already was. Aspalathus linearis is caffeine-free, a plain botanical fact that owes nothing to any of the above. It contains antioxidant compounds, among them aspalathin, which is found in no other plant. That is a matter of chemistry and worth knowing on its own terms, and it is a separate subject from the ERβ work described above; the estrogen finding is the actual result here, and antioxidants are not what produced it.

Dr Verhoog's own framing is the one the Ministry would adopt as policy: "Rooibos can be part of a healthy lifestyle, but it should not be seen as replacement for medical treatment."1

We will report back if and when the animal research produces something worth reporting, and we will say so just as plainly if it produces nothing. Until then, the cup on your table is what it has always been: red, caffeine-free, and nothing more than that.
